Azoximer bromide has a complex effect: immunomodulatory, detoxifying, antioxidant, moderate anti-inflammatory.

The basis of the mechanism of the immunomodulatory action of Azoximer bromide is a direct effect on phagocytic cells and natural killers, as well as stimulation of antibody formation and synthesis of interferon-alpha and interferon-gamma.

The detoxifying and antioxidant properties of Azoximer bromide are largely determined by the structure and high-molecular nature of the drug.

Azoximer bromide increases the body's resistance to local and generalized infections of bacterial, fungal and viral etiology. Restores immunity in secondary immunodeficiency states caused by various infections, injuries, complications after surgical operations.

A characteristic feature of Azoximer bromide when applied topically (sublingually) is the ability to activate the factors of early protection of the body against infection: the drug stimulates the bactericidal properties of neutrophils, macrophages, enhances their ability to absorb bacteria, increases the bactericidal properties of saliva and mucous secretions of the upper respiratory tract.

When administered orally, Azoximer bromide also activates lymphoid cells in the intestinal lymph nodes.

Azoximer bromide blocks soluble toxic substances and microparticles, has the ability to remove toxins, heavy metal salts from the body, inhibits lipid peroxidation, both by intercepting free radicals and by eliminating catalytically active Fe 2+ ions. Azoximer bromide reduces the inflammatory response by normalizing the synthesis of pro- and anti-inflammatory cytokines.

Azoximer bromide is well tolerated, has no mitogenic, polyclonal activity, antigenic properties, does not have an allergenic, mutagenic, embryotoxic, teratogenic and carcinogenic effect.

Azoximer bromide is odorless and tasteless, does not have a local irritating effect when applied to the mucous membranes of the nose and oropharynx.

Pharmacokinetics

Azoximer bromide after oral administration is rapidly absorbed from the gastrointestinal tract, the bioavailability of the drug when administered orally is more than 70%. The maximum plasma concentration is reached 3 hours after ingestion. The pharmacokinetics of azoximer bromide is linear (plasma concentration is proportional to the dose taken).

Azoximer bromide is a hydrophilic compound. The apparent volume of distribution is approximately 0.5 l / kg, which indicates that the drug is distributed mainly in the interstitial fluid. The half-life is 35 minutes, the half-life is 18 hours.

Indications for use

It is used in adults and children from 3 years old for the treatment and prevention of acute and chronic respiratory diseases in the stage of exacerbation and remission.

Suppositories Polyoxidonium

Candles 12 mg and 6 mg are administered rectally, after the bowel cleansing procedure, once a day (before bedtime). A single dose is one suppository. The scheme of their introduction is as follows: daily one at a time in the first 3 days, then - also one at a time - after 2 days. For a full course, 10 suppositories are required.

Intravaginal use is indicated for the treatment of gynecological diseases: dysplasia, erosion, leukoplakia of the cervix, colpitis, adnexitis, endometritis, as well as diseases provoked by human papillomavirus (HPV).

Treatment involves the daily administration of one suppository for three days with a dosage of the active substance of 12 mg, after which the suppositories continue to be administered every other day. The course is carried out using 10 suppositories. In cases where it is advisable, the course of treatment can be repeated after 3-4 months.

Lyophilisate

Injections in ampoules are intended for parenteral (intramuscular (IM) or intravenous (IV)), intranasal administration and sublingual administration in children. Solution preparation rules:

V / m administration: for adults - dissolve the contents of 1 vial (6 mg) in 1.5-2 ml of water for injection or 0.9% sodium chloride solution; for children - dissolve 3 mg of the drug in 1 ml of 0.9% sodium chloride solution or water for injection;

Intranasal administration: for adults - the contents of 1 bottle (6 mg), for children - ½ bottle (3 mg), should be dissolved in 1 ml of 0.9% sodium chloride solution, distilled or boiled water at room temperature.

One drop of the resulting solution for intranasal administration in children contains 0.15 mg of azoximer bromide. This solution is also used for sublingual use, it can be stored for no more than 7 days in the refrigerator, before use, the pipette is heated to room temperature.

Solution for parenteral administration should be prepared before direct use. The route of administration and dose is determined by the doctor based on clinical indications, taking into account the age of the patient. Recommended dosage for parenteral administration:

  • Tuberculosis: 6-12 mg 2 times a week, the course of treatment is 10-20 injections;
  • Correction of immunodeficiency after surgical removal of the tumor, radiation and chemotherapy, prevention of immunosuppressive action: 6-12 mg 1-2 times a week for a long period. In acute renal failure, the drug should be administered no more than 2 times a week.
  • Acute inflammatory pathologies: adults - 6 mg per day for 3 days, then - 1 time in 2 days, only 5-10 injections; children - at the rate of 0.1 mg per 1 kg of the child's weight per day, the procedure is carried out every other day, in total - 5-7 injections;
  • Rheumatoid arthritis: 6 mg every other day - 5 injections, then - 2 times a week, at least 10 injections in total;
  • Acute and chronic urogenital pathologies: 6 mg every other day, only 10 injections in combination with chemotherapy drugs; Chronic recurrent herpes: 6 mg every other day, course of therapy - 10 injections while prescribing antiviral agents, interferons and inducers of interferon synthesis;
  • Chemotherapy for oncological pathologies: 6-12 mg every other day, the course is at least 10 injections, then the frequency of administration is prescribed individually, taking into account the tolerability and duration of radiation and chemotherapy;
  • Complicated forms of allergic diseases: adults - 6 mg 1 time per day for 2 days, then - every other day, only 5 injections; children - intramuscularly at 0.1 mg per 1 kg of body weight per day after 1-2 days, in total - 5 injections in combination with basic therapy;
  • Inflammatory diseases of chronic origin: adults - 6 mg 1 time per day every other day (5 injections), then - 2 times a week, the course of treatment - at least 10 injections; children - 0.15 mg per 1 kg of weight every 3 days, the course of treatment - up to 10 injections;
  • Acute allergic and toxic-allergic conditions (in / in combination with antiallergic drugs): adults - 6-12 mg, children - 0.15 mg per 1 kg of weight;

Recommended dosing for intranasal administration:

  • Adults: 3 drops in each nasal passage 3 times a day for 5-10 days;
  • Children: 1-3 drops in one nasal passage 2-4 times a day.

To prepare a nasal solution, 6 mg of the lyophilizate is combined with one milliliter (20 drops) of distilled water, NaCl solution or ordinary boiled water. In this case, the temperature of the liquid should be at room temperature.

The solution is injected at least 3-4 times a day into each nasal passage, keeping 2-3 hours between instillations. A single dose is 1 to 3 drops in each nostril. Treatment is continued for five to ten days.

A lyophilisate for external topical use and injections is commercially available - mainly at a promotional price. But on the official website of the manufacturer, the instructions for it have not been available for several years, although references to this form have been preserved.

Possibly, the ambiguous situation is related to the 2006 recall episode of several batches of the Grippol vaccine, which also included azoximer bromide and the rights to which belong to its own manufacturer. Nearly 100 people throughout the Russian Federation, acute allergic reactions to the drug were detected, but no deaths were recorded.

The Rospotrebnadzor check confirmed the direct "participation" of the active substance Polyoxidonium in their development. And the lyophilisate implies its intramuscular injection similar to the vaccine. There is no publicly available data on its withdrawal from production or revocation of the license. Officially, it is believed that the drug can be used for children from six months, with autoimmune diseases, together with corticosteroids (immunosuppressors) - for all categories of patients, with the exception of:

  • allergic to azoximer bromide or excipients;
  • nursing mothers (due to the poor knowledge of its effect on the fetus and newborn);
  • patients with acute renal or hepatic insufficiency.

The instructions do not contain specific instructions for its use and action for hemorrhoids, hyperplasia, erosion and cervical cancer.

There are age restrictions on the use of individual forms:

  • tablets - not for children until the swallowing reflex is formed;
  • – do not prescribe before the 6-year threshold;
  • injections of lyophilizate intramuscularly/intravenously are also indicated from 6 years and older.

But because of the cases of complications mentioned above, there are doubts about how safe Polyoxidonium really is for immunity. Instructions for use of the drug take them into account with the vague wording "rarely", and contain an indication of possible soreness along the entire length of the muscle fiber after the injection.


And she describes candles and pills as completely safe. But a number of skeptical experts point to other possible threats.

  1. Cellular metabolism is 100% dependent on the normal structure and permeability of membranes. In particular, the high ability to change both parameters makes spread, a solid vegetable oil with an artificially added extra carbon chain, a carcinogenic product. If the azoximer bromide harms the metabolism of protective bodies in a similar way, it may be a catalyst for their malignant transformation. And the increase in the activity of resistance agents here is explained by the loss of functionality of a significant part of them (the bodies that have retained the ability to capture antigens are trying to compensate for their "disability").
  2. The autoimmune reactions provoked by it, judging by indirect and not systematized data, are more common than indicated in the instructions. And with existing pathologies of this type, it is more likely not to be taken than vice versa.

The long-term effects of taking Polyoxidonium are unclear. And in the short term, the mechanism of its action, which remains a mystery to science, is equally likely to be an innovation or a placebo.

Azoximer bromide does not inhibit isoenzymes CYP1A2, CYP2C9, CYP2C19, CYP2D6, cytochrome P-450, so the drug is compatible with antibiotics, antiviral, antifungal and antihistamines, glucocorticosteroids and cytostatics.
