The User (a capable natural person who has reached the age of 18 and uses the Site for personal, home and other needs) agrees with the processing of personal data provided by him to the Operator (Laboratory Electronic Medicine LLC) subject to the following conditions:
1.1. When registering on the Site and / or creating an Order (“Make an appointment”), the User provides the Operator with the following personal data (his own or as a full representative of the subject of personal data - close relatives):
- surname, name, patronymic, contact phone number, compulsory medical insurance policy number, e-mail address.
1.2. By providing his personal data when registering on the Site and / or creating an Order, the User gives the Operator consent to the collection, systematization, accumulation, storage, clarification (updating or changing), use, distribution, transfer to third parties, depersonalization, blocking and destruction of personal data ( or gives consent as a full representative of the subject of personal data).
1.3. The Operator uses personal data for the purpose of registering Users on the Site, providing services for making an appointment at a medical organization selected by the User from those available on the Site.
1.4. Consent is valid for 10 (ten) years from the date of the last Order or from the date of registration on the Site, whichever occurs later.
1.5. The Operator undertakes not to disclose personal data received from the User. It is not considered a violation of this obligation that the Operator provides personal data to medical organizations in order to fulfill obligations to Users. It is not considered a violation of obligations to disclose personal data in accordance with reasonable and applicable legal requirements.
1.6. The Operator has the right to use "cookies" technology and receive information about the User's ip-address. Cookies do not contain confidential information and are not used to identify the User.
1.7. In case of withdrawal of consent to the processing of personal data by the User, the Operator guarantees that all information received from the User, including login and password, is automatically deleted from the memory of the Operator's electronic computers, after which the User will not have access to the Site with their help. In order to withdraw consent to the processing of personal data, the User must delete his account in the "Personal Account" of the site.
