Nazivin– a drug in the form of nasal drops or spray, which has a pronounced anti-edematous and vasoconstrictor effect. The drug contains an active component - oxymetazoline - which stimulates alpha-adrenergic receptors located in the smooth muscle layer of the vessels of the nasal mucosa.   • Drops in the nose. They are available in glass bottles equipped with special pipettes on the cap. The concentration of such a drug can be 0.01%, 0.025% and 0.05%. The drug with the lowest dosage is intended for children under one year old and is presented in a 5 ml bottle. Drops with a concentration of 0.025% and 0.05% are available in 10 ml in one bottle. In addition to oxymetazoline, they contain disodium edetate, dihydrogen phosphate and sodium hydrogen phosphate, as well as water, sodium hydroxide and benzalkonium chloride.
  • Nasal spray. It is produced in plastic bottles that have a sprayer. Inside one bottle there is 10 ml of solution, and the concentration of oxymetazoline in it is 0.05%, which corresponds to a dosage of 500 µg/1 ml. Additional components of the medicine are citric acid, glycerol, benzalkonium chloride, purified water and sodium citrate.

Storage conditions and shelf life

It is recommended to keep Nazivin and Nazivin Sensitive at home at temperatures up to 25 degrees Celsius in a place where the medication will be securely hidden from small children. The shelf life of all forms of the medicine is 3 years. If it has expired, dripping or squirting the solution into the child’s nose is unacceptable. The date after which the medicine is no longer used is marked on the packaging of the spray or drops and does not depend on the time of opening.

Pharmacological properties

Pharmacokinetics

When administered locally intranasally, the drug does not have a systemic effect. After instillation into the nose, the effect of oxymetazoline occurs quickly:

For the 0.05% form for a few seconds (average 20 seconds)

For 0.025% and 0.01% forms in a few minutes

The duration of action of the drug is up to 12 hours. The half-life of oxymetazoline when administered intranasally is 35 hours. 2.1% of oxymetazoline is excreted in the urine and about 1.1% in the feces.

Pharmacodynamics

Nazivin ® has a vasoconstrictor effect, is a sympathomimetic, with a direct stimulating effect on alpha2-adrenergic receptors. When applied topically, nasal drops narrow the blood vessels of the nasal mucosa, eliminating swelling and hyperemia of the nasopharyngeal mucosa. Facilitates nasal breathing during rhinitis.

After the swelling of the mucous membrane is removed, the excretory channels open and expand for aeration of the paranasal sinuses and auditory tube. This stimulates the drainage function of the nasal cavities and prevents the development of bacterial complications. The antiviral, immunomodulatory and antioxidant effects of the active substance have been proven. The antiviral effect of oxymetazoline has been confirmed by studies using cultured cells infected with viruses (therapeutic approach). This causal mechanism of action has been demonstrated through inhibition of cold and runny nose viruses using the plaque reduction test, the residual viral infectivity test (viral titration), and the viral cytopathogenic effect inhibition (ZPE) test.

Indications for use

  • acute respiratory diseases with symptoms of rhinitis (runny nose)
  • allergic rhinitis
  • vasomotor rhinitis
  • improvement of secretion drainage during inflammation of the paranasal sinuses (sinusitis)
  • eustachitis associated with rhinitis
  • elimination of swelling of the nasal mucosa before diagnostic procedures

Directions for use and doses

Nazivin ® 0.01%, 0.025% and 0.05% drops are intended for intranasal use.

Nazivin ® 0.01% nasal drops used in infants under 4 weeks of age, 1 drop in each nasal passage 2-3 times a day. From the 5th week of life to 1 year - 1-2 drops in each nasal passage 2-3 times a day.

To ensure dosage accuracy, Nazivin bottle ® The 0.01% drops have a graduated pipette with drop number markings. For example, if 1 drop is prescribed, then the pipette should be filled with the solution to the 1 mark.

The following procedure has also been proven effective: depending on age, 1-2 drops of a 0.01% solution are applied to cotton wool and wiped over the nasal passages.

Nazivin ® 0.025% nasal drops prescribed to children aged 1 to 6 years, 1-2 drops in each nasal passage 2-3 times a day.

Nazivin ® 0.05% nasal drops prescribed for adults and children over 6 years old, 1-2 drops in each nasal passage 2-3 times a day

Nasal drops containing oxymetazoline should not be used for longer than 5-7 days unless otherwise directed by your doctor. Any repeated course of use of the drug must be preceded by a period without treatment of several days. Doses higher than recommended can only be used under medical supervision.

Single dose Nazivin ® 0.01%, 0.025% and 0.05% cannot be used more often than 3 times a day.

Side effects

Often(from ≥ 1% to< 10%):

  • burning sensation or dryness of the nasal mucosa
  • sneezing, especially in sensitive patients.

Infrequently(from ≥ 0.1% to< 1%):

  • swelling and edema of the mucous membrane (feeling of nasal congestion) may become more intense (reactive hyperemia), nosebleeds
  • hypersensitivity reactions (angioedema, rash, itching).

Rarely(from ≥ 0.01% to< 0,1%):

  • increased heart rate, increased pulse rate and increased blood pressure.

Very rarely < 0,01% и единичные случаи:

  • restlessness, insomnia, fatigue (drowsiness, sedation), headache, hallucinations (especially in children)
  • arrhythmias
  • apnea in infants and newborns
  • convulsions

Contraindications

  • hypersensitivity to oxymetazoline or to any excipient
  • atrophic rhinitis
  • Nazivin ® 0.025%, nasal drops should not be used in children under 1 year of age
  • Nazivin ® 0.05%, nasal drops should not be used in children under 6 years of age
  • cannot be used after transsphenoidal hypophysectomy or other surgical interventions on the dura mater

Drug interactions

Combined (simultaneous) use of oxymetazoline and

  • tricyclic antidepressants
  • tranylcypromine-type monoamine oxidase inhibitors
  • hypertensive drugs

may lead to increased blood pressure. Therefore, if possible, it is not advisable to use these drugs simultaneously.

special instructions

Treatment in newborns and infants must be carried out with extreme caution.

In the following cases, this drug should only be used after a careful risk/benefit assessment:

  • increased intraocular pressure, especially narrow-angle glaucoma
  • severe cardiovascular disease (eg, coronary disease) and hypertension
  • pheochromocytoma
  • metabolic disorders (eg, hyperthyroidism, diabetes mellitus)
  • prostatic hyperplasia
  • porphyria
  • patients being treated with monoamine oxidase inhibitors (MAO inhibitors) and other drugs that have the potential to increase blood pressure

With prolonged use of nasal remedies for the common cold or their overdose, their effectiveness may decrease. Improper use of nasal remedies for runny nose can cause:

  • reactive hyperemia (resumption of symptoms after discontinuation of the drug)
  • chronic swelling/edema of the nasal mucosa (rhinitis medicamentosa)
  • atrophy of the mucous membrane.

Pregnancy and lactation

When used during pregnancy or breastfeeding, do not exceed the recommended dosage. Data obtained from a limited number of women receiving this drug during the first trimester of pregnancy did not indicate the development of any adverse reactions due to oxymetazoline that could affect the pregnancy or the health of the fetus/newborn.

Features of the effect of the drug on the ability to drive a vehicle or potentially dangerous mechanisms

After long-term use of Nazivin ® , in dosages exceeding the recommended ones, a general effect on the cardiovascular system and central nervous system cannot be excluded. In these cases, the ability to operate a vehicle or equipment may be impaired.

Overdose

Symptoms: if recommended doses are exceeded or accidentally ingested, the following symptoms may appear: constriction of the pupils, nausea, vomiting, cyanosis, fever, tachycardia, arrhythmia, collapse, cardiac depression, arterial hypertension, pulmonary edema, respiratory disorders. In addition, mental disorders may appear, as well as depression of the functions of the central nervous system, accompanied by drowsiness, decreased body temperature, bradycardia, arterial hypotension, respiratory arrest and possible development of coma.

Treatment: taking activated carbon, gastric lavage, ventilation of the lungs with oxygen. To lower blood pressure - phentolamine 5 mg in saline (physiological) solution slowly IV or 100 mg orally. The use of vasopressor drugs is contraindicated. If necessary, prescribe medications to reduce fever symptoms and anticonvulsant therapy.

Release form and packaging

5 ml of the drug with a dosage of 0.01% in a dark glass bottle with a pipette cap. 10 ml of the drug with a dosage of 0.025% and 0.05% in a dark glass bottle with a pipette cap.

The bottle, along with instructions for medical use in the state and Russian languages, is placed in a cardboard box.
