Rhinoplasty is one of the most difficult operations in aesthetic medicine.

According to official statistics, complications after it occur in 7-13% of patients. They can be associated with both the negligence of the surgeon and the neglect of rehabilitation standards on the part of the patient.

In this material, I covered in detail the stages of rehabilitation and gave basic recommendations for protecting patients from side effects.

Recovery “after” depends on what is done “during” the operation.

Important There is no difference between closed and open rhinoplasty in terms of recovery!

When performing an operation using a closed technique, the skin of the nose is also peeled off from the soft tissues, and the same capillaries and vessels are also cut. If we are talking about rehabilitation, then here are clear examples of patients immediately after the removal of the cast.

As you can see, there is no difference between recovery after open and closed rhinoplasty.

Bruises and hematomas during surgery - what will the anesthesiologist say?

Many patients are afraid of bruising, swelling of the nose and around the eyes. Primary edema develops during surgical manipulations during surgery. If it is significant, it prevents the doctor from working efficiently and thoroughly with cartilage and soft tissues. Added to this is bleeding. The manifestation of these processes depends on the anesthesiologist, and not on the surgeon! During the operation and immediately before it, the anesthesiologist administers adrenaline-based drugs that cause transient vascular spasm.

A “dry surgical field” is an ideal option for a surgeon to consistently and correctly achieve the goals and objectives planned before rhinoplasty. And this is the merit of an intelligent anesthesiologist, working with whom is real luck.

Now remember how many times you heard about this from surgeons during consultations or read about it on the websites of different clinics? Good surgeons take pride in their team. The bad ones save on it.

The role of the plastic surgeon

During operations on the face, I use special medications to stop the development of edema. This step contributes to faster postoperative neutralization of congestion.

Important! The best way to protect yourself from complications is to choose not even a surgeon, but his team - an experienced and competent anesthesiologist, rehabilitation specialists and junior medical staff!

The duration and complexity of rehabilitation greatly depends on the algorithm of the doctor’s actions during the intervention.

Immediately after surgery

The nose itself at the end of the operation looks like this:


It looks creepy, but this blue-violet color disappears within 2-3 days, the patient does not see it - everything is hidden by a fixing bandage!

Any work of a plastic surgeon entails the formation of hematomas (the common name is bruises). Surgeons using outdated rafter techniques leave extensive blue-violet marks on patients' faces, localized not only on the nose, but also around the eyes. I perform rhinoplasty using modern and advanced technologies, so my patients are not afraid of their reflection in the mirror after surgery - There are no bruises around the eyes immediately after surgery! The exception is those cases when a person has increased fragility of blood vessels. In this situation, I recommend taking medications in advance that increase their elasticity and strength.


By the time the plaster cast is removed from the “new” nose, the cyanosis has already disappeared and the nose itself acquires its natural color. But hematomas under the eyes may last a little longer. Therefore, I advise you to undergo a series of special procedures to speed up the rehabilitation period (more on this below).

First 3 days

For the first three days, you will be able to breathe mainly through your mouth, because there will be special splints in the nasal passages, which, although they allow you to breathe, make this process difficult. They stop bleeding and maintain shape. Under no circumstances should you remove them yourself!

The first 7-10 days and subsequent period

During the first 10 days, a splint is placed on the nose - a special plaster cast or metal pad that restrains swelling and fixes the new shape.

After removing the bandage, the swelling intensifies. The main temporary problem will be difficulty breathing. There is no cause for concern: the swelling will go down and breathing will be restored. Swelling in deeper tissues takes quite a long time to subside, so a stabilized result can be assessed no earlier than after a year.

After plastic surgery, keep in constant contact with me and my assistants: 1-2 consultations in the first week, once after removing the plaster and scheduled examinations throughout the year.

Global (residual) edema

Swelling after rhinoplasty is a sore subject. It is known that they disappear from 4 to 12 months, depending on the individual characteristics of the body. The final result of the operation can be assessed only after the complete disappearance of congestion. Visually, they appear exclusively on the nose itself - it seems to you that the nose is a little swollen, sometimes it seems that the tip of the nose protrudes strongly and is itself larger than necessary.


The nose will take its final shape 8-12 months after the date of surgery. There is no need to knead it, massage it or soak it; swelling is a natural physiological process that we cannot speed up, unlike the disappearance of bruises.

Once again about the scar with the open technique

The topic of scars after open and closed rhinoplasty still causes heated debate in the professional and consumer environment.

Some surgeons have developed a special “fashion” - to promote closed rhinoplasty under the auspices of the absence of any traces of intervention. There is no point in arguing with the last factor - closed rhinoplasty really does not leave the slightest scar on the outer nose. But is this price too high for the fact that your nose will hardly change?

Please note that With closed rhinoplasty, the surgeon has extremely limited options for correction, so it won’t be possible to change your nose radically. For example, if we are talking about a hump, it will decrease by a maximum of 1.5-2 mm. It is not at all customary to operate on the tip of the nose with a closed approach - the work is too scrupulous, delicate and “jewelry” to be performed virtually blindly. To these shortcomings is added another important nuance - the number of surgeons who really know how to work in a closed technique can be counted on one hand.

After open rhinoplasty, in which I and the vast majority of my colleagues in Russia and the West work, a suture remains across the columella. By the time the plaster is removed it looks like this:


After posting this photo on Instagram, I received a lot of questions along the lines of “Where is the tripe?” from subscribers. The secret is that it is really difficult to notice a scar after an open rhinoplasty performed by a professional surgeon with the naked eye. And if within 10-14 days it is still visible in the form of a thin pinkish stripe, then after a month it completely merges with the surrounding skin in shade, structure and relief.

I have not met a single person who has dealt with a columella scar using laser resurfacing. Just because after 3-6 months, my patients themselves find it difficult to show where it was located.

Of course, there are exceptions here too. Surgeons who have poorly mastered the university or institute program make incisions and sutures carelessly, and therefore the wounds scar accordingly. They undertake to perform surgery on those who suffer from keloidosis. I think it is beneficial for them to promote exclusively closed rhinoplasty.

Rehabilitation after rhinoplasty can vary depending on the operation. For example, with a minor correction, the consequences of the surgeon’s work will be insignificant. In the case of complex rhinoplasty, when the entire surface of the face is affected, rehabilitation will last longer.

Duration

Recovery after rhinoplasty can take two to six months. The main factors influencing the duration are the method, complexity and quality of the operation performed.

Changes occur approximately every week. After the first week the swelling subsides, after two weeks you can use cosmetics, after a month all traces of the operation disappear.

Rehabilitation in the early postoperative period

After rhinoplasty is completed, the patient begins to wake up from anesthesia. The sleep effect is most often used, so the complexity of this part depends on the correct choice and calculation of medications. To minimize discomfort after surgery, the patient is prescribed premedication.

At this point in rehabilitation, a number of symptoms are observed:

  • Dizziness .
  • Nausea.
  • Weakness.
  • Constant desire sleep.

The above symptoms will go away immediately after the drugs wear off. To avoid irritation and inflammation and to prevent high fever, a number of antibiotics are usually taken after rhinoplasty.

The course of treatment is most often individual. In some cases, the patient also takes painkillers.

Fixation of the nose after surgery

Recovery after rhinoplasty is a difficult period, as you need to constantly monitor the condition of your nose. The slightest damage can cause serious damage to regenerating tissues.

To protect the nose during the recovery period, special fixing devices are used, the main ones of which are:

  • Plaster bandages (splint).
  • Thermoplastic.

Thermoplastic is more popular, since it does not need to be constantly adjusted to the decreasing swelling. You should also use special nose plugs during the recovery period.

They absorb tumor secretions and make recovery less uncomfortable. Nowadays, hemostats or silicone devices are used.

These clamps are removed within two weeks after surgery.

Rehabilitation in the late postoperative period

The first few weeks are the most difficult part of recovery. After several weeks of rehabilitation, the patient is no longer burdened by some of the restrictions associated with the operation.

Within a month, all visually visible marks disappear. After the swelling and bruising go away, the loss of sensitivity in the skin of the nose will also pass.

Whether the client follows the regimen or not will determine how long it takes for the nose to heal after rhinoplasty. To avoid complications after surgery, you need to follow some rules:

  • Dream only in a lying position face up.
  • Do without heavy loads and tilts.
  • Give up classes sports for the recovery period.
  • Don't go to solarium, or to the beach for two months.
  • Eat only moderate food temperature.
  • Do not wear glasses within three months.

The recovery process must be monitored by the attending physician; any restrictions are imposed or removed only with his permission.

It should be remembered that patients during the recovery period do not have external signs of the consequences of the operation after a month. But the tumors will disappear completely only after a few months or even six months.

Complete recovery may take a year. It all depends on the scale. The tip of the nose will heal faster than treatment after complex rhinoplasty.

The type of surgery will also affect the treatment. If the rhinoplasty was closed, the recovery will last up to six months. In the case where such a procedure was carried out openly, it will take more time for the nose itself to regenerate and the scar to disappear.

How to speed up recovery

The speed of recovery is directly related to the client's overall health. But there are still ways to speed up recovery.

In order to remove bruises after rhinoplasty, you should follow a diet that excludes alcohol and foods high in salt from your diet.

For a short time after surgery, it will become more difficult to breathe. This symptom is associated with drying out of the ichor on the respiratory tract (after mechanical intervention).

Under no circumstances should you try to get rid of dried ichor; it should peel off on its own. Otherwise, there is a very high probability of damaging the mucous membrane and thereby prolonging the recovery time.

Medicines after

Post-rhinoplasty medications such as Lyoton, Dimexide and Troxivazin will help speed up the disappearance of edema during the recovery process. Medicines must be prescribed by your doctor.

In order to get rid of swollen cartilage and swelling, it is recommended to massage the nose. This series of exercises is available for you to do independently:

  • Massaging tip nose with two fingers.
  • Massaging bridge of the nose two fingers.

Procedures should be performed several times a day, thirty seconds each.

Prohibitions

It may seem that the recovery will be very long and difficult: an abundance of bruises, plaster casts, difficulty breathing. In fact, this procedure is one of the most painless corrections. Unpleasant sensations pass very quickly, but every person should be aware that there are a number of contraindications for the rehabilitation period after rhinoplasty.

Basic prohibitions during the recovery period

  • Prohibited dream in any position except face up.
  • Carrying prohibited glasses, due to the risk of nasal deformation. For people with poor vision, it is recommended to wear lenses for three months after surgery.
  • No heavy ones loads
  • You can't take it hot bathroom or shower.
  • Refusal of any kind solar baths for one to two months.
  • None swimming pool during two months
  • You should try to limit yourself from getting sick cold or any similar disease that irritates and affects the mucous membrane.

It is a very common fear that rhinoplasty surgery may leave permanent scars on the face. This is wrong. In both open and closed surgery, the skin on the main part of the face and nose is not subjected to any kind of damage.

The only part of the skin where any traces will remain is the septum between the nostrils, but even on it, with proper care, there will be no trace of surgical intervention.

  • An independent decision to carry out this operation is possible only when coming of age, otherwise, with the written permission of the parents.
  • It is worth changing the shape of the nose only in certain cases. borders otherwise the tip plate may not hold up.

The main stages of rehabilitation after plastic surgery

After the procedure, severe swelling of the face occurs; in addition, open rhinoplasty is performed under general anesthesia, which causes serious health problems. The most difficult and responsible time is the postoperative period, which lasts 1-4 weeks, depending on the degree of tissue damage and the regenerative ability of the body. The task of this period is to eliminate the consequences of anesthesia, properly restore soft and cartilaginous tissue in the operated area, and protect the weakened body from infectious infection. Photos taken daily during the first two weeks help the doctor record changes in the patient’s condition. Every day after surgery is of significant importance for the formation of its result:

Day 1: immediately after the operation and suturing, turundas (cotton tourniquets) are installed in the nasal passages, and a plaster or plastic splint is attached to the nose to protect the tissues formed by the surgeon from physical impact. The patient is in a sleepy state; side effects of anesthesia from other systems may develop. Without appropriate medication, severe pain, nosebleeds, and broken blood vessels in the skin and whites of the eyes are likely.

6-8 days: the splints are replaced, the sutures are removed after open surgery. Nasal breathing is partially restored, the hematomas begin to resolve.

10-14th day: the fixing bandage is finally removed, the patient is allowed to return to work. The condition of the nose immediately after removing the splint is far from ideal, but due to the swelling, it is too early to judge the result of plastic surgery.

The second stage begins after removal of the fixation splint and lasts from two to six months. This period is necessary for complete resorption of swelling and strengthening of soft tissues. The doctor prescribes various procedures that accelerate regeneration or eliminate minor cosmetic defects. Upon completion, you can draw initial conclusions about the success of the operation.

The third stage of rehabilitation after rhinoplasty involves the patient’s complete return to an active lifestyle and takes up to a year. Complications after rhinoplasty

As with any surgical intervention, various complications may inevitably arise after plastic surgery:

1. Allergy to anesthesia. With general anesthesia it leads to the death of one person in 50,000 people operated on.

2. Stopping nasal breathing. At a normal rate of rehabilitation, he recovers after a week. If surgery has been performed to remove a nasal hump, nasal breathing may be permanently impaired.

3. Decreased sensitivity, numbness of the nose, upper lip, dysfunction of the sense of smell. It goes away as the innervation of soft tissues is restored.

4. Scars on the lower part of the nose. They can be removed through cosmetic procedures or corrected by a surgeon.

5. Infectious contamination. It is very dangerous for a weakened body; a course of antibiotics is prescribed for prevention.

After closed rhinoplasty, complications are much less, and recovery is faster. Unfortunately, the applicability of such an operation is limited, and the result is less accurate.

How to shorten the rehabilitation period and avoid complications?

To speed up your recovery, you need to follow a few simple dos and don’ts:

1. Do not touch the fixing bandages and turundas installed by the surgeon; until they are completely removed, avoid any physical contact with the nose. This includes not attempting to wash your nose or clear your nasal passages. During the first week of rehabilitation, it is better to breathe only through your mouth.

2. You need to train yourself to sleep on your back. Sleeping on your stomach is unacceptable until the end of rehabilitation, on your side for the first 3 months.

3. To reduce swelling after rhinoplasty, limit fluid and salt intake. After removing the splint, in consultation with your doctor, you can use various ointments or sign up for physiotherapeutic procedures.

4. During the first 2 months of rehabilitation, it is necessary to avoid situations that can cause nosebleeds. In particular, you need to sleep on a high pillow, not tilt your head, avoid contrasting water procedures, too hot or cold food, intense physical activity, and sunbathing. For the first 2 weeks, it is recommended to avoid severe anxiety, take a mild laxative and eat foods rich in fiber to combat constipation. You can engage in contact sports or sports involving high physical stress no earlier than 6 months after rhinoplasty.

5. Protect yourself from colds for 3 months, avoid swimming pools and ponds. During this period, you should not sneeze or blow your nose, this will lead to bleeding, and in the worst case, to deformation of the nose.

6. It is forbidden to wear glasses for 3 months; constant load on the bridge of the nose can affect the process of its formation.

7. Smoking and drinking alcohol are prohibited until the swelling disappears completely (3-6 months).
