The eye is a very complex organ, consisting of several types of tissues and various elements. With the help of visual organs, a person interacts with the outside world. However, the eyes are prone to various kinds of inflammatory diseases, injuries and infections.

Inflammation can occur both in any part of the eye itself, and in the periorbital zone. Infections are caused by viruses, bacteria, and fungi. Traumatic processes are the result of the penetration of a foreign object, such as sand. Chemicals can also be the cause of inflammation.

Ophthalmic diseases are manifested in the form of redness, itching, tearing, pain, photophobia, dryness, swelling. When alarming symptoms appear, urgent measures must be taken so that the disease does not worsen, otherwise it may lead to loss of vision. In this case, Oftan Dexamethasone eye drops can come to the rescue. The next article will be devoted to instructions for the use of this drug.

What it is?

Oftan Dexamethasone eye drops is an ophthalmic agent with anti-inflammatory and anti-allergic effects. The medicinal substance belongs to the group of glucocorticosteroids.

Dexamethasone is presented as an active ingredient. The active substance of the drug interacts with protein receptors that are located in target tissues. It does not allow leukocyte membranes to collapse, and also reduces cell migration and division.

The active ingredient contributes to a violation of the recognition of antigens by the immune system. All this leads to the suppression of inflammation that occurs in response to mechanical, chemical and immune irritation.

IMPORTANT! After instillation of one drop of the drug, the anti-inflammatory effect persists for four to eight hours.

Why are they prescribed

Eye drops are prescribed for acute and chronic inflammatory processes, namely:

  • uveitis, iritis, iridocyclitis;
  • blepharitis, keratitis, conjunctivitis, keratoconjunctivitis;
  • scleritis, episcleritis;
  • choroiditis.

Oftan Dexamethasone can also be used for allergic eye diseases. The tool is used for both therapeutic and prophylactic purposes. It prevents the occurrence of an inflammatory reaction after surgery and mechanical injury.

Instructions for use

In acute inflammatory processes, one or two drops are instilled into the conjunctival sac every two hours. After reducing the pathological process, it is enough to use the drug three to five times a day. The duration of the treatment process should not exceed two to three weeks.

If after three to five days a positive result is not obtained, it is necessary to contact a specialist to adjust the treatment. Perhaps the patient will be prescribed an additional pharmacological agent.

Consider the steps for using Oftan Dexamethasone eye drops:

  1. Wash your hands with soap and then dry.
  2. Open the vial with the solution with clean, dry hands. Shake well before use.
  3. Take a position that is comfortable for you. You can lean on the back of a chair or lie down on a sofa. Changing the position of the body does not affect the quality of instillation of eye drops.
  4. Tilt your head back.
  5. Using your index finger, gently pull back the lower eyelid so that a crease forms between the eyelid and the eye. Don't overdo it. If the eyelid is pulled back too much, the medicine may simply leak out without having the desired therapeutic effect.
  6. Look slightly up, while trying not to lose sight of the bottle of medicine. Try to bring it as close to the eye as possible, but do not touch the mucous membrane. Careless movements can lead to injury or infection.
  7. Drop the required amount of the drug into the conjunctival sac. If necessary, repeat the steps with the second eye.
  8. Close your eye and lightly press on the inner corner. This will help prevent leakage of the solution from the conjunctival sac into the nasal cavity. Such a measure will ensure the complete flow of the drug into the tissues of the eye.
  9. Close the vial and place it in the refrigerator.

In acute inflammatory processes, Oftan Dexamethasone eye drops are instilled every two hours.

Using a mirror will help control your movements and avoid injury. If there is a need to instill other eye drops, observe the fifteen-minute interval. Contact lenses must be removed before the procedure. You can put them on again no earlier than twenty minutes after installation.

Do not think that the result will be stronger if you independently increase the dosage of eye drops. The conjunctival sac will still not contain a larger amount of treatment solution. By doing so, you can cause overdose symptoms to appear.

Precautionary measures

Eye drops are contraindicated in such cases:

  • eye tuberculosis;
  • fungal or viral infection;
  • infections caused by mycobacteria;
  • glaucoma - increased intraocular pressure;
  • exacerbation of purulent ophthalmic diseases;
  • corneal damage.

Oftan Dexamethasone is also not prescribed to children, pregnant women, nursing mothers. If the doctor decides to treat the drug during the period of bearing a child, the duration of the solution should not exceed ten days. Individual hypersensitivity to the active ingredient is also a limitation to the use of eye drops.

The drug is well tolerated. Nevertheless, sometimes symptoms of local irritation may appear in the form of itching, pain, redness, sensation of the presence of a foreign body. Many patients note that immediately after the installation, blurred vision and a burning sensation appear. However, these unpleasant symptoms quickly disappear.

Treatment with Oftan Dexamethasone can lead to other adverse reactions:

  • profuse lacrimation;
  • increased sensitivity to light;
  • coloration of the cornea;
  • double vision;
  • drooping of the upper eyelid (ptosis);
  • headache, dizziness;
  • change in taste sensations;
  • the appearance of scales on the edges of the eyelids.

Prolonged use of the drug can lead to an increase in intraocular pressure. Hypertension of the visual organs is fraught with disruption in the work of the optic nerve, up to its damage, deterioration in visual acuity, narrowing of peripheral vision. This all creates ideal conditions for the development of glaucoma, a dangerous disease that can cause blindness. Also, subject to long-term treatment, there are risks of cataracts, perforation of the sclera and ulceration of the cornea.

CAREFULLY! Simultaneous use of Oftan-Dexamethasone with mydriatics (drugs that dilate the pupil) can lead to an increase in intraocular pressure.

The preservative benzalkonium chloride is present as auxiliary components in the solution. It can cause discoloration of contact lenses.

Treatment with corticosteroids may blur the picture of a bacterial or fungal infection. If a combined pathology has been diagnosed, other drops with antimicrobial activity are prescribed along with Oftan Dexamethasone.

In case of an overdose of Oftan Dexamethasone (when used locally), the occurrence of systemic side effects is unlikely. If the therapeutic dosage is exceeded, it is recommended to wash the excess drug under warm running water.

In no case should treatment be interrupted when you feel better. To consolidate the result, you should complete the full course prescribed by your doctor. Patients who use Oftan Dexamethasone for a long time are regularly checked for IOP (intraocular pressure) indicators.

Analogues

In some cases, the doctor may decide to prescribe a similar drug with similar pharmacological properties. Consider the most popular drugs:

  • Hydrocortisone. Available in the form of an ophthalmic ointment. Relieves inflammation, swelling, as well as an allergic reaction. The basis of the drug is synthetic hormonal components.
  • Maxidex. The active substance is dexamethasone. It is most often used for ophthalmic inflammations of an allergic nature.
  • Pharmadex. The active ingredient of the drug belongs to the group of corticosteroids. Used in the treatment of inflammatory and allergic pathologies of the conjunctiva, cornea, anterior segment of the eye, sensitive to steroids.
  • Dexapos. It is used for severe inflammation of the eyes of a non-infectious nature, including allergic reactions.

pharmachologic effect
Pharmacodynamics

Synthetic fluorinated glucocorticosteroid. It has a pronounced anti-inflammatory and anti-allergic effect.

Interacting with a specific protein receptor in target tissues, it regulates the expression of corticoid-dependent genes and thus affects protein synthesis. Stabilizes lysosomal enzymes of leukocyte membranes; inhibits the synthesis of kinins, mitosis and migration of leukocytes; inhibits antibody synthesis and impairs antigen recognition. All these effects are involved in the suppression of the inflammatory response in tissues in response to mechanical, chemical or immune damage. The duration of the anti-inflammatory action after instillation of 1 drop of the solution is from 4 to 8 hours.

Pharmacokinetics
When applied topically, systemic absorption is low. After instillation into the conjunctival sac, it penetrates well into the epithelium of the cornea and conjunctiva; at the same time, therapeutic concentrations are achieved in the aqueous humor of the eye; with inflammation or damage to the mucous membrane, the rate of penetration increases. About 60-70% of dexamethasone entering the systemic circulation binds to plasma proteins. Metabolized in the liver by cytochrome P450 containing enzymes (CYP3A4); metabolites are excreted through the intestines. The plasma half-life (t1/2) averages 3.6 ± 0.9 hours.

Indications for use
Acute and chronic inflammatory processes:

Non-purulent forms of conjunctivitis, keratitis, keratoconjunctivitis without damage to the epithelium and blepharitis;
- scleritis and episcleritis;
- iritis, iridocyclitis and other uveitis of various origins;
- superficial damage to the cornea of various etiologies (chemical, physical or immune mechanisms) after complete epithelization of the cornea;
- inflammation of the posterior segment of the eye (choroiditis, chorioretinitis);
- sympathetic ophthalmia.

Allergic eye diseases:

Allergic conjunctivitis or keratoconjunctivitis.

Prevention and treatment of inflammatory phenomena in the postoperative and post-traumatic period.

Contraindications

Keratitis associated with herpes simplex, chickenpox and other viral diseases of the cornea and conjunctiva;
- eye tuberculosis;
- mycobacterial eye infections;
- fungal diseases of the eyes;
- acute purulent eye diseases;
- damage to the epithelium of the cornea (including the condition after removal of a foreign body of the cornea);
- corneal epitheliopathy;
- increased intraocular pressure;
- glaucoma;
- children under 18 years of age (due to the lack of data on the efficacy and safety of the drug in children and adolescents under 18 years of age);
- hypersensitivity to any of the components of the drug.

Pregnancy and lactation
To date, there are no data from clinical trials regarding the use of Oftan® Dxamethasone during pregnancy and lactation. Oftan Dsksamstazon can be used during pregnancy and while breastfeeding only on the prescription of the attending physician, if only the expected therapeutic effect justifies the potential risk to the fetus and to the child.
Duration of therapy: no more than 7-10 days.

Dosage and administration
In acute conditions: 1-2 drops in the conjunctival sac every 1-2 hours.

After reducing inflammation, Oftan® Dsksamstazon eye drops are instilled 1-2 drops into the conjunctival sac 3-5 times a day.

The duration of treatment with Oftan® Dexamethasone eye drops should not exceed 2-3 weeks.

The decision on the duration of treatment is based on objective data, including the effectiveness of the drug, the severity of clinical symptoms and the possible risk of side effects.

Side effect
After instillation of Oftan® Dexamethasone eye drops, a quickly passing burning sensation, allergic reactions may occur.

Long-term use (more than 3 weeks) of dexamethasone can cause secondary glaucoma and steroid cataracts, as well as ulceration, clouding, thinning or perforation of the cornea; in rare cases, dexamethasone can contribute to the spread of herpes and bacterial infections.

Oftan® Dexamethasone contains the preservative benzalkonium chloride, which may cause eye irritation.

In case of side effects, you should contact your doctor as soon as possible.

Overdose
Overdose with topical application of Oftan® Dexamethasone eye drops is unlikely.
Symptoms: local irritation is possible.
There is no specific antidote. The drug should be discontinued and symptomatic therapy prescribed.

Interaction with other drugs
Interaction with other drugs is mainly due to the participation of cytochrome P450-containing enzymes (CYP3A4) in the excretion of dexamethasone. It induces the CYP3A4 enzyme, thus reducing the effectiveness of calcium channel blockers, quinidine and erythromycin. With the usual regimen of topical application, the dose of the drug is insufficient to cause induction or saturation of hepatic enzymes.
With prolonged use with iodoxuridine, it is possible to increase the destructive processes in the corneal epithelium.

special instructions
Oftan® Dexamethasone contains the preservative benzalkoi chloride, which can be absorbed by soft contact lenses and cause discoloration and adverse effects on eye tissue. If it is necessary to use contact lenses during treatment with Oftan® Dexamethasone, they should be removed before using the drug and, if necessary, put on no earlier than 15 minutes after instillation.

During treatment with the drug for more than 2 weeks, it is necessary to regularly monitor intraocular pressure and the condition of the cornea.

Corticosteroid therapy may mask an ongoing bacterial or fungal infection. In the presence of infection, the use of drops should be combined with appropriate antimicrobial therapy.

In view of the possible lacrimation after instillation, the drug is not recommended to be used immediately before driving vehicles or operating mechanical equipment.

In medicine, Oftan Dexamethasone eye drops act as a highly effective drug that belongs to the group of steroid hormones from the subclass of corticosteroids produced by the adrenal cortex. In ophthalmology, Oftan Dexamethason is applied topically and is strictly contraindicated for oral administration. Like most medications, it has a list of contraindications for use and can cause side effects, therefore, in order to successfully complete the treatment, you must first obtain medical advice and read the annotation.

Components and principle of operation

The drug "Oftan Dexamethasone" is sold in the form of a solution that has neither color nor odor. The drug contains the active ingredient dexamethasone sodium phosphate and additional ingredients such as:

  • distilled water;
  • monobasic Lewis acid;
  • "borax";
  • benzalkonium chloride;
  • trilon b.

Oftan Dexamethason eye drops act locally and practically do not enter the general bloodstream. After instillation, it quickly and easily penetrates into the stratum corneum and connective membrane of the eye, where it has a pronounced anti-allergic and anti-inflammatory effect. The duration of the therapeutic effect of drops varies within 4-8 hours.

Inhibition of the inflammatory reaction in the affected tissue structures of the visual organ is achieved by disrupting the antigen recognition process with dexamethasone and slowing down the formation of antibodies.

When are they assigned?

The drug is often used for inflammation localized on the mucous membrane of the organ.

Ophthalmic drops "Oftan Dexamethasone" are recommended for the treatment of inflammatory processes, the localization of which is the edges of the eyelids, the cornea, the iris of the eyeball and the mucous membrane of the eye. A medicine is used to eliminate superficial damage to the cornea and eliminate the inflammatory focus from the posterior segment of the visual organ. In addition, "Oftan Dexamethasone" is an excellent prevention of the development of inflammatory phenomena after surgery and injuries.

Oftan Dexamethason eye drops should be used as prescribed by a specialized ophthalmologist, who will write out the dosing regimen individually for each patient, depending on the diagnosis and severity of his condition. If the doctor has not prescribed personal treatment, then an ophthalmic solution should be used, referring to the instructions, which describe that in the acute form of the disease "Oftan Dexamethasone" you need to drip 1-2 drops into both conjunctival sacs every 1-2 hours. As the inflammatory focus decreases, the frequency of instillations is reduced to 5 times a day. The instructions for use describe that the duration of treatment is usually 2-3 weeks, however, at the discretion of the doctor, these terms can be changed depending on the severity of the symptoms and the effectiveness of the therapy.

Application restrictions

It is best to avoid drug therapy in patients with glaucoma.

You should not resort to Oftan Dexamethasone therapy for patients who have been diagnosed with the following pathologies:
