Pharmacological action

Indications for use

Disinfection of the oral cavity and pharynx for inflammatory and infectious diseases- tonsillitis, pharyngitis, tonsillitis-pharyngitis, tonsillitis, glossitis, aphthae; when preparing a patient for surgery and postoperative period; for the treatment of oral infections arising during chemotherapy, as an additional agent to antibiotic therapy for sore throats caused by streptococci, in influenza conditions in the prodromal period.

Contraindications

The drug is contraindicated in hypersensitivity to iodine or other components of the drug, with hyperfunction thyroid gland, with decompensated heart failure, with renal failure, it is contraindicated to use the drug 2 weeks before and after studies or radioactive iodine therapy.

The active substance penetrates the placental barrier and penetrates into mother's milk Therefore, the drug is contraindicated during pregnancy and breastfeeding.

Yox spray is contraindicated in children under 8 years of age.

Pregnancy and lactation

Pregnancy

There is no experience with the topical use of povidone-iodine in the oral cavity of pregnant women. At vaginal use povidone-iodine, or local application on the skin during pregnancy or during childbirth, congenital hyperthyroidism or struma was found in newborns, therefore the use of Yox during pregnancy is not recommended.

Lactation

Iodine passes into mother's milk. It was found several times in the blood and urine of newborns more iodine than their mothers who used povidone-iodine. High serum concentrations may cause hypothyroidism in neonates. For this reason, Yox is contraindicated for use during lactation.

Directions for use and doses

Adults and children over 8 years old:

Usually applied 2-4 times a day, if necessary, more often (every 4 hours): 1-3 injections into the right and left side oral cavity or directly to the affected area. The interval between individual applications of the drug should be at least 4 hours. The drug can be used a maximum of 6 times per day. The dose of the drug is determined by the attending physician. The applicator is washed with hot water before and after use.

Side effect

The drug is well tolerated.

In some cases, hypersensitivity reactions to the drug are observed, local manifestations are possible allergic reaction on iodine (itching, hyperemia, urticaria), which requires discontinuation of the drug. Rarely, mainly in children, momentary sensation burning or feeling of dryness in the mouth.

Overdose

There is no experience of overdose with proper local use in the mouth or pharynx.

When using iodine solution orally, acute iodine poisoning is possible.

Symptoms: Initially, patients notice a metallic taste in the mouth, vomiting, pain in the stomach and stool disorders. Within 1 - 2 days, anuria and swelling of the glottis manifests itself, leading to asphyxia, aspiration pneumonia or pulmonary edema. In some cases, vascular insufficiency was observed.

Treatment is purely symptomatic; first, standard measures are taken to stop absorption from gastrointestinal tract. The victim is given milk and boiled starch. If the esophagus is not affected, then gastric lavage can be performed. Then he is appointed activated carbon and 1% or 5% sodium thiosulfate solution, which reduces iodine to iodide. The elimination of absorbed substances can be accelerated by osmotic diuresis. Experience with dialysis in such situations has not been described in the literature.

Interaction with other drugs

Inappropriate simultaneous use Preparationfn Yox with others antiseptics, intended for local O use in the mouth and throat, especially those containing hydrogen peroxide.

The use of Iox leads to false-positive results of stool analysis for occult blood. When studying the function of the thyroid gland, values may increase protein bound Yoda.

Features of application

For laryngitis, the drug can be used only if absolutely necessary.

The drug should be used with caution in patients with liver failure. In patients with liver failure, the level of transaminases may increase, so it is necessary to monitor the level of these enzymes in the blood serum.

Impact on the ability to drive a car and other mechanisms The drug Yox does not affect the ability to drive a car or use other machinery. The drug contains 19% ethanol.

In elderly patients and in patients with liver failure, the dosage does not change.

Name:

Jox

Pharmacological
action:

Combined drug with antiseptic effect .
The drug contains polyvidone-iodine and allantoin.
Iodine has a pronounced bactericidal effect, exhibits a moderate immunostimulating effect, has the property of increasing the secretion of mucus by excretory cells of the mucous membranes, thus providing a mucolytic effect, and has proteolytic properties.
Polyvidone is a high molecular weight compound used as an iodine carrier.
Thanks to its presence, it decreases toxic effect iodine to the cells of the body.
When iodine is released, it changes quaternary structure protein, reduces the catalytic activity of proteins and enzymes of the capsid shells of viruses, the replicative system of bacteria and protozoa. The presence of a high-molecular carrier ensures faster and easier penetration of iodine into cells, ensures high concentrations antiseptic in the cage a short time after administration.

Polyvidone-iodine has a bactericidal effect on microorganisms of the following types:: Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus pyogenes, Streptococcus faecalis, Neisseria sicci, Proteus mirabillis, Bacillus cereus, Candida albicans, Sacchoromyces cerevisiae, Enterobacter cloacae and other strains of gram-positive and gram-negative bacteria, fungi, viruses and protozoa.
The drug has a pronounced antimicrobial effect at respiratory diseases, as it easily penetrates into the deep layers of the epithelium of the mucous membranes of the respiratory tract, which gives quick effect not only for acute but also for chronic forms diseases.

Allantoin, which is part of the drug, has an anti-inflammatory and reparative effect, enhancing the effect of polyvidone-iodine.
In addition to its ability to inhibit bacterial growth, allantoin also softens the mucous membrane, relieving irritation and stimulating cell regeneration.
When applied topically, polyvidone-iodine better adsorbed through mucous membranes and damaged skin than through healthy skin.
As a result of penetration large quantity iodine into the body may interfere with thyroid functions.
In the body, iodine is metabolized to iodides and can accumulate in thyroid gland. It is excreted mainly by the kidneys, in small quantities with feces, saliva and sweat.
Iodides penetrate well through the hematoplacental barrier and are excreted in breast milk.

Indications for
application:

Sore throat (catarrhal, follicular, lacunar, caused by strains of microorganisms sensitive to the drug) is recommended complex therapy with oral or injectable antibiotics;

Insert the applicator tube into oral cavity, hold your breath and press 2 times, each time directing a stream of the drug to the corresponding part of the throat cavity on the right and left.
When using, the bottle must be held vertically.
Do not inhale or swallow the sprayed solution!
The oral applicator must be rinsed before use. warm water and check it for damage.
A damaged applicator must not be used.
Use in adults and children over 8 years of age, usually 2–4 times a day, if necessary, every 4 hours.
The spray can be applied no more than 6 times a day.
Elderly patients and patients with liver failure should undergo dose adjustment.
The duration of treatment depends on the nature and course of the disease and is determined by the doctor individually.

Throat rinse, concentrate.
The diluted solution is applied topically to the mucous membrane of the oral cavity and pharynx for rinsing or washing.
The drug should not be swallowed.
Yox concentrate is diluted drinking water in a ratio of 1:20–1:40 (namely: 2.5–5 ml using a measuring cap or ½ teaspoon per 100 ml of water).
Rinse or rinse the mouth and pharynx with 10–15 ml of diluted solution for about 30 seconds.
Use in adults and children over 6 years of age 2–4 times a day, no more than 6 times a day, with an interval between two applications of at least 4 hours.
Elderly patients are shown the same doses as adults.
There are no data on specific geriatric cases with topical iodine use.
The duration of treatment depends on the nature and course of the disease, which is determined by the doctor.

Side effects:

The drug is generally well tolerated. Possible occurrence allergic reactions for iodine (itching, hyperemia, urticaria, dry mouth). These reactions are usually transient. Symptoms of hypersensitivity occurred very rarely.
Side effects are listed below by frequency of occurrence: very often (≥1/10); often (≥1/100,<1/10); нечасто (≥1/1000, <1/100); редко (≥1/10 000, <1/1000); очень редко (<1/10 000), включая отдельные сообщения.
From the immune system: very rarely - symptoms of hypersensitivity, anaphylactic shock.
From the outside digestive system : uncommon - dry mouth.

From the skin and subcutaneous tissues: rarely - allergic reactions to iodine may occur, which require discontinuation of the drug - itching, hyperemia, urticaria; very rarely - angioedema.
General disorders and reactions at the injection site: rarely - a burning sensation at the site of application (especially in children), a feeling of dry mouth.
Injuries, poisoning and complications during procedures: iodism (metallic taste in the mouth, increased salivation, swelling of the eyes, larynx, lungs, skin rash, gastrointestinal disorders, systemic side effects in the form of metabolic acidosis, hypernatremia, impaired renal function, diarrhea) after long-term use.

Contraindications:

Hypersensitivity to iodine and other components of the drug;
- Dühring's dermatitis herpetiformis;
- hyperthyroidism, adenoma or thyroid dysfunction;
- decompensated heart and kidney failure;
- 2 weeks before and after examination or therapy with radioactive iodine;
- in the form of a throat rinse, concentrate - for children under 6 years of age;
- in spray form - for children under 8 years of age;
- pregnancy and lactation period.

Preparation not intended for oral administration: Do not inhale or swallow the drug. If the patient’s condition does not improve within 3 days or if an undesirable effect or unusual sensations are detected, a doctor’s consultation is necessary.
It is not advisable to simultaneously use the drug with other antiseptic drugs intended for use in the mouth and throat.
For laryngitis, the drug can be used only in exceptional cases.
The drug contains 18.6% ethanol and does not contain sugar.
Use with caution in patients with liver failure. In such patients, the level of transaminases may increase, so it is necessary to monitor their levels in the blood plasma.

The use of iodinated povidone may interfere with thyroid scintigraphy.
Should not be used iodized povidone for 1–2 weeks before thyroid scintigraphy.
Since it is impossible to exclude the development of hyperthyroidism, long-term (more than 14 days) use of iodized povidone or use in large quantities in patients (especially elderly) with latent thyroid dysfunction is permissible only after a careful assessment of the expected benefits and possible risks.
The patient's condition should be monitored to identify early signs of hyperthyroidism and properly evaluate thyroid function, even after discontinuation of the drug (for up to 3 months).
Do not inhale or swallow the spray solution! Avoid getting the drug in your eyes! If the drug gets into the eye, rinse it with water.

The drug can change the color of the oral mucosa, teeth, skin and clothing. Stains on the mucous membrane, teeth and skin can be removed with alcohol.
Stains on clothing can be removed by washing and rinsing with diluted ammonia (ammonia), sodium thiosulfate, or soap and water.
If possible, dentures and similar structures should be removed from the mouth before using the drug.
The drug should not be heated before use.
Children. Solution: The drug is used to treat children over 6 years of age.
Spray: used to treat children over 8 years of age.
In children aged 8–12 years, the drug is used under medical supervision. In children, the drug should be used with caution, given the possibility of laryngospasm.
Impact on the ability to drive vehicles or other machinery.
No effect.

Interaction with
other medicinal
by other means:

It is not recommended to combine the drug with other antiseptics for the oral cavity and pharynx, primarily with hydrogen peroxide.
Due to its oxidizing properties, iodized povidone may affect the results of some diagnostic tests such as detecting occult blood in stool or urine or glucose in urine.
The use of the drug leads to a false positive hemotest result.
May interfere with the results of some diagnostic tests (eg, thyroid scintigraphy, protein-bound iodine, radioiodine measurements) or may compete with iodine used in thyroid therapy.
The use of iodized povidone should be avoided in patients taking lithium preparations, since their simultaneous use may cause a synergistic hypothyroid effect.

Povidone iodized should not be used simultaneously with drugs containing chlorhexidine, silver sulfadiazine, due to possible partial inactivity.
Iodized povidone interacts with strong alkalis, sodium thiosulfate, sodium metabisulfite and thiomersal, therefore the drug should not be used simultaneously with these substances.
All interactions are manifested visually by discoloration of the solution, which indicates a decrease in the effectiveness of the drug.
The drug should not be used simultaneously with disinfectants containing mercury; ointments containing enzymes, or benzoin tincture; the drug is incompatible with oxidizing agents, alkali salts and acidic substances.

Pregnancy:

There is no experience with topical use of iodized povidone in pregnant women.
Congenital hypothyroidism or goiter have been reported in newborns whose mothers used topical iodine (either vaginally before birth or applied to the skin during pregnancy).
Therefore povidone is iodized cannot be used during pregnancy.
Iodine can pass into breast milk.
The detected iodine concentrations in the blood and urine of breastfed children were many times higher than the concentrations observed in their mothers who used iodized povidone.
High concentrations of iodine in blood plasma can cause the development of congenital hypothyroidism in children who are breastfed, therefore povidone is iodized cannot be used during breastfeeding.

Overdose:

With proper local use of the drug in the oral cavity or pharynx, no cases of overdose were observed.
Symptoms: when swallowing iodine solution, acute poisoning developed.
Patients first felt a metallic taste in the mouth, vomiting, stomach pain and diarrhea began. Within 1–3 days, anuria, swelling of the glottis followed by asphyxia, aspiration pneumonia or pulmonary edema were detected. In some cases, even severe circulatory disorders were noted.
Treatment: Symptomatic and initially includes standard measures to prevent further absorption into the gastrointestinal tract. The patient needs to drink milk and brewed starch (to brew starch, put 2 g of starch in a porcelain cup and mix well with 8 ml of cool purified water. Add 90 ml of boiling purified water. Stirring constantly, heat the mixture to a boil (0.5–1 min) and keep until it thickens and becomes lighter. Cool to room temperature (25 ° C) and take. If the esophagus is damaged, you can do a gastric lavage. In addition, you need to take activated charcoal and 1-5% sodium thiosulfate solution. , ensuring the reduction of iodine to iodide. There is no specific antidote.
