The drug "Cordiamin" belongs to analeptic drugs that stimulate the activity of the central nervous system.

This substance acts non-selectively.

The medicine performs various tasks, including activating the work of chemoreceptors.

The medication is able to influence the respiratory center of the brain, the vasomotor center located in the medulla oblongata.

Pharmacological action

Cordiamine simplifies the process of interaction between neuron cells during the transmission of nerve signals. Thanks to this, the excitability of the respiratory center to natural stimuli becomes higher.

The drug can also suspend inhibition processes, thereby giving the nerve impulse more time to act.

By acting on the vasomotor center, the drug increases peripheral vascular resistance, which causes blood pressure to become higher. However, the drug has no direct effect on the heart.

By activating the work of chemoreceptors located in the carotid sinus area, the drug is able to increase the number of respiratory movements. That is why Cordiamin has peripheral abilities.

The substance is an antagonist to the action of narcotic drugs (hypnotics). Can be used in case of overdose or poisoning.

The drug increases the number of respiratory movements, promotes deep breathing (replacing shallow breathing), normalizes blood pressure, and returns consciousness.

The substance is quite well absorbed by the body, and this process does not depend on the method of administration of the drug.

Time of onset of therapeutic effect:

  • with injection - on average after 3 minutes and lasts a little more than an hour (with IV) and a little more than 2 hours (with IM);
  • when taken orally – after 15 minutes and lasts about 3 hours;
  • In the liver, the drug takes the form of inactive metabolites, which are excreted by the renal apparatus.

Directions for use

Adult patients should receive the product in the amount of 15 to 40 drops up to 3 times a day. For injection (under the skin, into veins, into muscles), the amount of product should be 1-2 ml.

For patients childhood Drops are prescribed according to the age of the child. They are taken up to 3 times a day. If injections are given, then 0.1 to 0.75 ml of medication is administered twice or thrice a day.

Maximum dosage for an adult patient is 2 ml of the drug at a time and 6 ml per day.

In case of drug poisoning, a single dose can be about 5 ml.

When injecting the drug into the veins, the process should be as slow as possible.

The medicine in the form of drops can be taken regardless of meals.

To relieve discomfort (pain) during administration, you can pre-administer procaine.

Pharmacological properties

The effect of the drug Cordiamine, which helps with low blood pressure, is due to two main components of the mechanisms: peripheral and central. The peripheral mechanism is characterized by an increase in the frequency and depth of respiratory movements. The central mechanism has a direct effect on the vasomotor center of the medulla oblongata, which consists of increasing pressure and excitation of the center. It should be noted that the use of Cordiamin does not affect the cardiovascular system of the body.

For hypotensive patients, doctors can prescribe medications that sharply increase blood pressure and normalize their general condition. Such drugs include Cordiamine, whose international name is Nikethamide. This is an effective respiratory analeptic that relieves shock, asphyxia, etc. We will find out how it affects blood pressure further.

What kind of drug is this?

Cordiamine is an aqueous solution of the active ingredient, which is niketamide. There are no additional substances in the drug. So, Niketamide is specially dissolved in purified water, the concentration of which per 1 ml of product is 250 ml, that is, it is a 25% solution, which is a transparent liquid, but its slight light yellow tint is allowed. Has a specific strong smell.

The drug is a cardiac stimulant and does not have a tonic effect on the heart muscle and blood vessels.

How does it affect blood pressure: does it increase or decrease?

The active substance of the drug acts on the central nervous system, performing the following tasks:

  • irritates individual nerve nodes;
  • stimulates the respiratory center, and breathing movements acquire a deeper character;
  • activates impulses entering the brain;
  • constricts blood vessels, affecting the vasomotor center of the medulla oblongata.

Thanks to its complex action, the drug strongly and quickly increases blood pressure, therefore it is used for hypotension.

It is worth adding that the peripheral mechanism of action of the drug is characterized by its effect on the human respiratory system. Thanks to this, the frequency and depth of breathing increases, and the blood and internal organs are more quickly saturated with oxygen.

Release forms

Cordiamine is available in three types:

  • Solution for internal use. Available in bottles with darkened glass and different volumes - 25 or 30 ml.
  • Drops for internal use. Also available in bottles with darkened glass and the same volume. It differs from the previous form by the presence of a nozzle, which makes dosing the drug more convenient.

The two forms are no different in composition. They are produced by several pharmaceutical companies that have patented the drug under different names - “drops” and “solution”.

  • . Available in ampoules with a capacity of 1 and 2 ml. The package contains 10 ampoules.

Indications for use

Experts prescribe the drug for breathing problems and a sharp drop in blood pressure. These symptoms may be caused by the following:

  • Collapse is a condition that threatens a person’s life. It is characterized by a drop in blood pressure, resulting in a deterioration in the blood supply to vital organs.
  • Poisoning and overdose of narcotic substances, sleeping pills, analgesics and drugs that depress the central nervous system (for example, barbiturates).
  • Weakness of breathing or circulatory disorders in the body due to the development of severe infections.
  • State of shock (traumatic, anaphylactic,). Shock is a critical condition of the body in which organs stop working properly and lead to a severe circulatory disorder.
  • Suffocation or asphyxia, when oxygen starvation occurs and carbon dioxide accumulates in excess quantities in the blood and organs. Asphyxia occurs when the airways are compressed from the outside, closing their lumen, for example, with Quincke's edema.
  • Cardiovascular diseases associated with disruption of the heart.
  • Vascular insufficiency.
  • Respiratory dysfunction during or after surgery.

Instructions for use

Typically, drops and solution for internal use are used in the same way, since they are the same drug, but from different pharmaceutical companies. It is measured in drops. The dosage depends on the age of the patient:

  • for adults the maximum number of drops per day is 180, and 15–40 drops are allowed to be taken at a time, maximum 60;
  • for children under 10 years old a single dose corresponds to their age and is taken once a day, for example, a 6-year-old child is given 6 drops of Cordiamine, and a 9-year-old child is given 9;
  • for children over 10 years old and teenagers under 16 years old a single dose is 10 drops, and can be taken only once a day.

The drug is taken in its pure form, but can be diluted with water and then drunk. It can be taken at any time, that is, it does not depend on meals.

The duration of the course depends on the normalization of the condition and is determined by the reason that caused the decrease in blood pressure. In case of suffocation, it is enough to take it occasionally, and in case of an infectious disease, surgery or shock, the course lasts several days until complete restoration of functions.

Injections are administered intramuscularly, subcutaneously or intravenously, and the dosage also depends on the age of the patient:

  • adults inject 1-2 ml, maximum dose - 6 ml;
  • children and teenagers inject from 0.1 to 0.75 ml of solution (the younger the child, the less medicine is required).

When administered intravenously or subcutaneously, it is advisable to inject into the veins in the arms, slowly injecting the medicine. An intramuscular injection should be given in the thigh area. Since the administration of the medicine is accompanied by severe pain, the injection site should first be treated with Novocaine.

When treated with a solution, you should not drive a car or operate complex mechanisms that require increased attention, concentration, and psychomotor reactions.

Contraindications

The drug is not prescribed:

  • People who are prone to seizures:
  • people suffering from epilepsy;
  • children with elevated body temperature;
  • in case of individual intolerance to the component.

Separately, it is worth clarifying that pregnancy is not a contraindication to the use of the solution, but its constant use is undesirable, since the effect it has on the fetus has not yet been fully studied. So, if a pregnant woman experiences a sudden increase in blood pressure, the woman can take 30 drops of the solution to relieve the attack and prevent fainting.

Side effects

When taking the drug, a convulsive syndrome is often observed, which is due to the facilitation of interneuron transmission of nerve impulses. As a rule, this is manifested by twitching of the eyelids and lips. The following phenomena are less common:

  • allergy;
  • contraction of facial muscles;
  • redness of the skin, especially the limbs and face;
  • heart rate failure ();
  • digestive system disorder;
  • worry, anxiety.

Interaction with other drugs

Cordiamine should not be taken in parallel with vitamin C (ascorbic acid), since they are incompatible. In addition, it is undesirable to combine the drug with the following medications:

  • Sleeping pills, narcotic analgesics, antiepileptic drugs. Cordiamine reduces their effect.
  • Antidepressants, central nervous system stimulants, Reserpine. Cordiamine increases their effect, which can trigger seizures.

Analogs

These are those drugs that contain the active component Cordiamin. These are Niketamide, Nicotinic acid diethylamide, Codymide. Pharmacies also offer analogues in tablets - Securinin nitrate, Etimizol, Armanor. All of these medications also sharply increase blood pressure, so taking them requires extreme caution.

Cordiamine greatly increases blood pressure. This is a potent drug, so if uncontrolled self-medication it can cause harm. In case of overdose, there is a high probability of developing seizures. Be sure to follow the recommendations of a specialist when consuming.

Pharmacodynamics.

Mixed action analeptic. The mechanism of action consists of central and peripheral effects on the nervous system. The central mechanism is associated with a direct effect on the vasomotor center of the medulla oblongata, leading to its excitation and an indirect increase in blood pressure (especially when this center is initially depressed). The peripheral effect is due to the stimulation of the chemoreceptors of the carotid sinus, which leads to an increase in the frequency and depth of respiratory movements. Does not have a direct effect on the cardiovascular system.

Pharmacokinetics.

Absorption is high and does not depend on the route of administration. Subject to rapid metabolism in the liver with the formation of inactive metabolites, excreted mainly by the kidneys.

Indications for use:

Collapse, asphyxia (including newborns), shock during surgical operations and in the postoperative period, respiratory and circulatory depression in infectious diseases.

Contraindications:

Hypersensitivity to the components of the drug;

Decreased seizure threshold, seizures (including history);

Hyperthermia in children;

Pregnancy, lactation period.

Use during pregnancy and breastfeeding

There is no reliable data on the safety of using the drug during pregnancy and lactation. It is not recommended to use this drug during pregnancy and breastfeeding.

Dosage regimen, method of administration.

The drug is administered intravenously, intramuscularly and subcutaneously.

Adults and children over 14 years of age are administered subcutaneously, intramuscularly and intravenously in a dose of 1-2 ml 1-3 times a day.

Intravenous administration is carried out slowly.

For children - subcutaneously, depending on age, in a dose of 0.1-0.75 ml.

Since subcutaneous and intramuscular injections of the drug Cordiamin are painful, to reduce pain, depending on the situation, procaine (Novocaine) 1 ml of 0.5% solution is first injected into the site of the intended injection. Higher doses for adults subcutaneously: single - 2 ml, daily - 6 ml.
